1. Acceptance of Terms
By accessing or using our services, you agree to be bound by these Terms and Conditions. If you do not agree, you may not use our services.
2. Services Provided
Sandusky Wellness Center, LLC (the “Center”) provides chiropractic, nurse practitioner, and wellness services to patients in northern Ohio. Services are delivered by the Center’s licensed clinicians — including chiropractors, nurse practitioners, and other qualified wellness providers — and may include manual chiropractic care, examinations and evaluations, nurse practitioner visits, Shockwave therapy, and additional wellness modalities offered by the Center from time to time. The roster of providers and the specific services offered may change without prior notice.
3. SMS Messaging Terms (A2P / 10DLC Compliance)
By providing your phone number and opting in, you consent to receive SMS messages from Sandusky Wellness Center, LLC, which may include:

- Customer support messages
- Marketing or promotional messages (if applicable)

11. Medical Disclaimer and Informed Consent
Chiropractic adjustments, manual therapies, and wellness services provided by the Center are intended to support general health and musculoskeletal function. They are not a substitute for emergency medical care, primary medical care, or treatment of any specific disease or condition. If you are experiencing a medical emergency, call 911 or proceed to the nearest emergency department.
As with any healthcare service, results vary from patient to patient and no specific outcome is promised or guaranteed. Recommended treatment plans, frequency of visits, and adjunct services (including Shockwave therapy and other wellness modalities) are based on clinical judgment and the individual patient’s presentation.
Chiropractic care carries risks that may include, but are not limited to: temporary soreness, stiffness, bruising, dizziness, and — in rare cases — more serious complications. By accepting care, you acknowledge that the nature of chiropractic and wellness treatment, along with the material risks and reasonable alternatives, will be explained to you, and that you have the right to ask questions and to withdraw consent at any time.
Patients are responsible for providing complete and accurate health history information and for promptly notifying the Center of any change in condition, medication, or pregnancy status.
12. HIPAA and Protected Health Information
Sandusky Wellness Center, LLC is a covered entity under the Health Insurance Portability and Accountability Act of 1996 (“HIPAA”). The Center maintains a separate Notice of Privacy Practices that describes in detail how protected health information (“PHI”) is used and disclosed, and the rights you have with respect to your information. The Notice of Privacy Practices is available upon request and at the reception desk.
SMS and email channels are convenience tools and are not intended for the transmission of detailed clinical information. The Center will use these channels only for limited information such as appointment reminders, scheduling, billing notices, and general practice updates. Patients should not transmit clinical details, treatment questions, or any sensitive PHI by SMS or unsecured email. By initiating such communications, the patient acknowledges that standard mobile carrier and internet channels are not encrypted end-to-end and accepts the associated risks.
The Center will not sell PHI and will not use PHI for marketing purposes except as expressly permitted by HIPAA and applicable Ohio law.
13. Appointments, Cancellations, and No-Shows
Patients are expected to arrive on time for scheduled appointments. Late arrivals may result in a shortened visit or the need to reschedule, at the provider’s discretion, in order to protect the schedules of other patients.
The Center requests at least twenty-four (24) hours’ notice for cancellations or rescheduling. Cancellations made with less than 24 hours’ notice, and missed appointments without notice (“no-shows”), may be subject to a fee as posted at the front desk or otherwise communicated at the time of scheduling. Repeated late cancellations or no-shows may result in the Center declining to schedule future appointments.
Appointments for prepaid packages, Shockwave therapy series, and similar bundled services follow the same cancellation policy. Missed visits within a prepaid package will count against the package unless the Center, in its sole discretion, determines that rescheduling is appropriate.
14. Payment, Insurance, and Refunds
Payment for services is due at the time of service unless other arrangements have been made in writing. The Center accepts cash, check, and major credit and debit cards. Returned checks and chargebacks may be subject to an administrative fee.
As a courtesy, the Center may bill participating insurance carriers on the patient’s behalf. The patient is ultimately responsible for all charges, including any amounts not covered by insurance, applied to deductibles, copayments, coinsurance, or denied for any reason. Patients are responsible for understanding their own benefits, in-network status, and any prior authorization requirements.
Cash-pay services and prepaid treatment packages, including adjunct services such as Shockwave therapy, are generally non-refundable once treatment has commenced. The Center recognizes, however, that circumstances such as a change in medical condition, a contraindication identified after treatment has begun, or a documented relocation may warrant an exception. The Center will consider refund or credit requests in those situations in good faith and on a case-by-case basis. Where a full refund is not appropriate, unused portions of a prepaid package will, where reasonable, be credited toward other services rendered at the Center. All refund or credit requests should be submitted in writing — by letter or email — to the contact information listed in Section 10, and the Center will respond within a reasonable period of time.
Accounts that remain unpaid beyond thirty (30) days from the statement date may be subject to interest, collection costs, and reasonable attorneys’ fees, to the extent permitted by Ohio law, and may be referred to a third-party collection agency.
15. Governing Law and Dispute Resolution
These Terms and Conditions, and any dispute arising out of or relating to them or to the services provided by Sandusky Wellness Center, LLC, are governed by the laws of the State of Ohio, without regard to its conflict-of-laws principles. The parties consent to the exclusive jurisdiction and venue of the state and federal courts located in Erie County, Ohio for any action or proceeding not otherwise required to be resolved through an applicable administrative or licensing process.
Before initiating any formal legal action, the parties agree to attempt in good faith to resolve any dispute through direct discussion with the Center’s management.
16. Severability and Entire Agreement
If any provision of these Terms and Conditions is held to be invalid or unenforceable by a court of competent jurisdiction, the remaining provisions will continue in full force and effect. The invalid or unenforceable provision will be interpreted, or replaced by an enforceable provision, in a manner that most closely reflects the original intent of the parties.
These Terms and Conditions, together with the Center’s Notice of Privacy Practices, posted financial policies, and any written treatment or consent forms signed by the patient, constitute the entire agreement between the patient and Sandusky Wellness Center, LLC with respect to the subject matter hereof and supersede any prior or contemporaneous understandings, whether written or oral.
